Membership Application Form

This is the application for Union membership. You will need to fill this out either with your steward or the local president. Your filled-in form will be forwarded to your assigned regional office. Your information will be added to the union’s membership database and you will be sent a permanent membership card.

Local Membership Expense Claim Form

Any expense claim to the local should be accompanied with one of these.
